Please complete the application below to reserve your place at one of our seminars. After completion of the application, click "submit" and you will be directed to the PayPal payment page. The Vendor Fee is $200.00 for one table space and electrical. After payment is submitted, you will  be sent a confirmation via the email you provided. Please specify below if you have other specific needs so we can accommodate you.
We are sorry we can not accept any orthodic companies at our seminars because we have an                                                  exclusive contract with Foot Levelers only.
